Documentation Requred for LD:For DSS, generally, staff will look for the following: 1) that verification of your condition should be made by a qualified treating professional (psychiatrist, psychologist, MSW, etc.); and 2) Your diagnosis and how it affects major life functions with or without treatment. We prefer information about the educational impact of your impairment and academic accommodation recommendations. Ask your treating professional to give us a report that includes: 1. DSM-IV Diagnosis (date of diagnosis and most recent contacts), including the results of assessments used to make the diagnosis. 2. Description of how the condition affects major life activities. 3. Recommendations for academic accommodations (special testing, notetakers, adapted texts, etc.). DSS staff will work with you individually to acquire your documentation and determine your eligibility. Documentation needs to be from within the three-year reevaluation period. For the Achieve Program, the applicant must submit previous psychoeducational testing, from the earliest to the most recent, regardless of its age. Diagnostic information should include measures of cognitive ability, academic ability, and other appropriate measures (language, processing, executive functioning, etc.).

For Achieve, documentation should also include measures of cognitive ability as well.
