Documentation Requred for LD:Psychological-educational evaluations completed within last 3 years. Copy of latest IEP. (WAIS-III or IV as well as achievement tests, such as WJ-III or WIAT.)
Documentation Required for ADHD:In addition to the above documents, the student must submit a report from a neuropsychologist/neuropsychiatrist that clearly diagnoses AD/HD. Testing must no older than three years. List of current medications is also required.
