Documentation Requred for LD:It is essential that current typed documentation of a disability include specific information in a signed report from an appropriately qualified professional. Documentation must include the appropriate information as listed below. 1. A comprehensive diagnosis or assessment of a medical or psychological condition that is defined as a disability under ADA, 2. A statement as to how the disability substantially limits the student’s ability to engage in major life activities, as defined by ADA, and that the student’s current functional limitations are the direct result of the disability, 3. A comprehensive description of the assessment methods by which these limitations were determined, 4. The date of the assessment of the disability and determination of the student’s current functional limitations, and 5. The credentials and signature of the professional who is providing the diagnosis or assessment 6. Mail or personally deliver a copy of the original documentation to the Coordinator of Student Disability Services at DePauw University, 115 Asbury Hall, 100 E. Seminary Street Greencastle, IN 46135-0037
